Raising Voices: Advocates and Policymakers Share Strategies for Families and Providers
Even with overflowing plates, families and providers can have an impact on the issues affecting the care and support of medically complex individuals. In this breakout session, panelists from the Pennsylvania Health Law Project (PHLP), Little Lobbyists, BAYADA Hearts for Home Care, and more will provide information on advocating directly for your loved ones and patients, as well advocacy efforts and opportunities at the state and national level. Q & A to follow.

Partners in Person-Centered Care: Tools and Resources for Providers & Caregivers to Improve Care at Home
There is much work to be done on a systems-level to increase the home care workforce and improve the care experience for families across the state - but families, nurses, and agencies can work together now to become stronger partners in care! In this session PA Families Need Nurses Now will share information about a Family Council pilot program that helps agencies, nurses, and families understand each other’s perspective and partner more effectively to shape the care being delivered by the agency and nurses in the homes. We will also share our “Beyond the 485” template and discuss the benefits of a care plan that focuses on the whole child. Q & A to follow.
